Submit a Personal Data Request
You may submit the following types of requests:
- Opt-Out of Selling or Sharing Personal Information
- A request that Aflac record your preference to not share your personal information with third parties or use your personal information for certain behavioral advertising.
- Limit the Use of My Sensitive Personal Information
- A request that Aflac record your preference to limit the use and disclosure of certain sensitive personal information about you to the performance of certain services.

Aflac CCPA Mail-in Instructions
The privacy rights that are available through this process are only available to California state residents.
To submit and receive your request through the mail:
- The type of request being submitted and provide contact information including name, phone number, address, and whether you are a California resident or not.
- Allow up to 45 days for Aflac to process the request and this may be extended by an additional 45 days if more time is needed. If an extension occurs, Aflac will inform you within the initial 45-day window.
- Mail the request to:
- For Aflac: Aflac, Attn: Privacy Office, 1932 Wynnton Road, Columbus, GA 31999.
- For Aflac Group / CAIC: Aflac Group, Attn: Privacy Office, PO Box 427, Columbia, SC 29202.
If submitting this request on behalf of someone else, in addition to the above, please also include:
- Written permission signed by the consumer authorizing you to act on their behalf.
- Your contact information, including your name, phone number, and address to send communications to.
