And, you may submit a request via mail using instructions below.
Aflac CCPA Mail-in Instructions
The privacy rights that are available through this process are only available to California state residents.
To submit and receive your request through the mail:
- Proof of Identity (copy of the front and back of an Acceptable Proof of Identity)
- Acceptable Proof of Identity is State Issued ID / Driver’s License or Passport or Passport Card.
- The type of request being submitted and provide contact information including name, phone number, address, and whether you are a California resident or not.
- Allow up to 45 days for Aflac to process the request and this may be extended by an additional 45 days if more time is needed. If an extension occurs, Aflac will inform you within the initial 45-day window.
- Mail the request to:
- For Aflac: Aflac, Attn: Privacy Office, 1932 Wynnton Road, Columbus, GA 31999.
- For Aflac Group / CAIC: Aflac Group, Attn: Privacy Office, PO Box 427, Columbia, SC 29202.
If submitting this request on behalf of someone else, in addition to the above, please also include:
- Power of Attorney or another official document that authorizes you to act on behalf of the Data Subject.
- Proof of Identity of the third-party individual making the request (copy of the front and back of an Acceptable Proof of Identify).
- Acceptable Proof of Identity is State Issued ID / Driver’s License or Passport or Passport Card.
- Copy of proof of registration with the Secretary of State (if you represent a business entity).
- Contact information, including your name, phone number, and address to send communications to.
