Consent to Electronic Transactions
Your acceptance of this Consent to Electronic Transactions is evidenced by your clicking “I Agree” and decision to continue to apply for Aflac coverage through Aflac.com (“the Website”). In doing so, you further acknowledge and accept that you have read, understand, and agree to the terms and conditions of this Consent to Electronic Transactions. If you do not agree to this Consent to Electronic Transactions, do not proceed with applying for Aflac coverage online. You may also apply for Aflac coverage by calling 800-992-3522 or reaching out to an Aflac agent through “Find an Agent” on the Website.
Please read this information carefully and print a copy and/or retain this information electronically for future reference.
This Consent to Electronic Transactions concerns your rights to consent to utilize electronic transactions and electronic records through or on the Website. It also concerns your right to consent to receiving electronic versions of certain records to which you are legally entitled and the use of electronic signatures. In order to complete an application for Aflac coverage through the Website, we need your consent. This document informs you of your rights when conducting electronic transactions and receiving electronic records from Aflac and vendors with whom we do business.
Consent to Electronic Transactions, Signatures and Communications. By applying for Aflac coverage on the Website, and regardless of whether the application process is completed, you consent to the use of electronic communications, electronic transactions, and electronic signatures in connection with your Aflac policies, contracts, and/or accounts, and further consent to receive by electronic means various notices, documents, and correspondence from Aflac, and/or brokers, agents or third party providers acting on Aflac’s behalf, and other materials regarding our products, to the extent available and permitted by law (which may include, but not be limited to, statements, invoices, claim correspondence, contracts, explanations of benefits, and other materials that Aflac is, or may be, legally required to deliver to you) (collectively, “Communications”).
Electronic Communications. Aflac will use reasonable means to protect the security and confidentiality of information sent and received by email. However, Aflac cannot guarantee the security and confidentiality of email communications.
Email Address. You have the sole responsibility for providing Aflac with a correct and operational email address. Aflac will not be liable for any undelivered email communications or any costs you incur for maintaining Internet access and an electronic mail account. You must promptly notify us of any change in your email address by contacting 800-992-3522.
If you do not have an email address and provide us with the email address of another person who will forward any email communications to you, you do so at your own risk. Further, if you provide Aflac with an email address that is accessed by multiple users, you do so at your own risk. Aflac will not be liable for any harm related to the use by anyone else of your email address.
E-Mail, Hardware, and Software Requirements. You are responsible for accessing, opening and reading any documents and/or electronic communications that apply to your application for coverage through Aflac.com. You may need to make certain changes to your e-mail account so that you can receive e-mails from Aflac. Please consult your e-mail provider for assistance in making these changes so that e-mail or spam filters do not prevent you from receiving incoming e-mails from the aflac.com domain. To access documents and/or communications that apply to your application for coverage through Aflac.com electronically, you must meet the following minimum hardware and software requirements: internet access, Internet Explorer 6.0 or better, Adobe Acrobat Reader 4.0 or better, and a valid e-mail account. If you cannot access or read the documents related to your application for Aflac coverage, you must contact Aflac immediately at 800-992-3522. Further, to print or download any policy documents and/or communications, you should have a printer or appropriate computer storage or storage device.
To Obtain Paper Copies. You understand that you can obtain a paper copy of your policy documents and/or communications by printing it yourself or by obtaining a paper copy by mail from Aflac. You may obtain a paper copy of a particular policy document or communication at no charge by calling 800-992-3522 and requesting a copy.
Mobile Text Alerts. By including your mobile phone number in your application for Aflac coverage, you are certifying that you are the account holder for the mobile phone account or have the account holder’s permission to use the mobile phone number to receive information about your application for Aflac coverage. You are also consenting to receive alerts from Aflac or an Aflac agent via text messages. Text message fees may apply. Text messages may be transmitted automatically. Aflac will only send text messages permitted by law, such as transactional or fraud alerts.
Message frequency varies by account and preferences. Message and data rates may apply. You may opt out at any time by following the instructions, if required to be provided, on the text message received from Aflac.
