Hospital Intensive Care Claims
To file a hospital intensive care claim, please complete the appropriate claim form and follow the guidelines below:
Please remember to submit a signed and dated authorization with all information provided.
- Include an authorization signed and dated by the patient with every claim.
- Send a copy of your hospital bill showing the charges and the number of days you were in intensive care. Your claim cannot be processed without the hospital bill.
- Have your physician complete and sign Section B: Physician's information.
- Send a copy of a police accident report if your claim is a result of a motor vehicle accident.
