The Policyholder Service Center will allow you to:
- Make an address change.
- Request duplicate copies of your policy.
- Request an ID card.
- Request your policy status.
These additional requests may require a signature on forms mailed to you:
- Bank draft change
- Beneficiary change or inquiry
- Name changes
- Policy reinstatement request
- Spouse/child addition or deletion
If your inquiry involves one of the following, please e-mail us directly:
- Checking the receipt of a claim
- Claim status
- Policy benefits
- Claim forms
Note: If your premiums are paid through payroll deduction on a pre-tax basis (Section 125 Plan), notify your benefits administrator or payroll specialist before making any changes.
