The Policyholder Service Center will allow you to:

  • Make an address change.
  • Request duplicate copies of your policy.
  • Request an ID card.
  • Request your policy status.

These additional requests may require a signature on forms mailed to you:

  • Bank draft change
  • Beneficiary change or inquiry
  • Name changes
  • Policy reinstatement request
  • Spouse/child addition or deletion

If your inquiry involves one of the following, please e-mail us directly:

  • Checking the receipt of a claim
  • Claim status
  • Policy benefits
  • Claim forms

Note: If your premiums are paid through payroll deduction on a pre-tax basis (Section 125 Plan), notify your benefits administrator or payroll specialist before making any changes.
